Latest call · Q2 2026

high-risk, watch rather than buy after the call.

Spero has secured $105 million of nonrecourse royalty financing and expects funding into the second half of 2029, but it reported zero Q2 revenue and a $9.6 million net loss while its new investment case rests on an early-stage, largely unvalidated SP001 program. The bull case is Utebzi commercialization by GSK and potential differentiation in IgG4-related disease; the key tension is that proof-of-concept data are still years away and the financing carries 10% annual interest.

Near term

GSK expects Utebzi to become available to U.S. patients in the second half of 2026; launch timing and early uptake are the most immediate value catalysts.

The $35 million nonrefundable upfront payment to Innovent will consume part of the financing proceeds, despite management's stated runway into the second half of 2029.

Investors will need evidence that Utebzi royalties can support the financing structure and ultimately generate meaningful retained cash flow for Spero.

Longer term

SP001 Phase II in IgG4-related disease is expected to begin in Q2 2027, with only up to 15 patients in each of two open-label dosing arms; the design may establish an initial signal but is unlikely to resolve efficacy or durability questions.

The commercial opportunity is unproven. Management cited 20,000–40,000 diagnosed U.S. patients and potential diagnosis expansion, but provided no market-share, pricing, or revenue framework.

SP001's proposed advantage over B-cell depletion is mechanistic rather than clinically demonstrated. Its positioning alongside or after Uplizna, rituximab, steroids, and other therapies remains uncertain.

The company is transitioning from a completed antibiotic development program to immunology, making execution, regulatory alignment, and clinical-development capabilities important structural risks.

Red flags

There is currently no clinical proof that SP001 provides durable disease control, reduces steroid use, or improves outcomes in IgG4-related disease; the supporting data cited were Phase I/Phase Ib studies, including healthy volunteers and primary Sjogren's disease.

Management declined to comment on Uplizna's uptake, leaving competitive intensity and the likely treatment position for SP001 unquantified.

Claims that SP001 could be best-in-class were not supported with comparative clinical data; management's response cited PK, specificity, and preclinical features only.

The royalty financing is nonrecourse to corporate assets, but its 10% interest rate and quarterly principal and interest payments could materially reduce the economic value of future Utebzi royalties.

Zero Q2 revenue and a $9.6 million quarterly net loss underscore that the company remains dependent on future royalties, financing economics, and successful clinical execution rather than current operating revenue.
