IRIDEX delivered strong Cyclo G6 probe growth—17,700 units, up 35%, and $3.9 million of G6 revenue, up 19%—but total revenue fell 7% to $12.6 million, retina revenue dropped to $6.5 million from $8.0 million, and adjusted EBITDA was negative $0.4 million. The bull case is improving glaucoma utilization and future margin gains from outsourcing; the key tension is that the $51–$53 million FY2026 revenue target requires a much stronger second half despite distributor timing, regulatory delays, competitive console pressure, and only $4.7 million of cash.
The second-half revenue recovery is the immediate test: management expects low-single-digit retina growth and low-double-digit glaucoma growth, with Q4 historically the strongest quarter.
Cyclo G6 probe demand remains the clearest positive indicator, but management acknowledged Japan and Europe placed heavier orders ahead of blackout periods; the next quarter could show a corresponding inventory-driven decline.
Cyclo G6 system placements fell to 18 from 35 year over year. A rebound in Europe, the Middle East and Africa and stronger Q4 capital-equipment sales are necessary to support the outlook.
Cash generation may be pressured in Q3 by safety-stock purchases. Although management reported positive operating cash flow, cash increased only $0.1 million to $4.7 million.
The company reaffirmed FY2026 revenue guidance of $51 million–$53 million, excluding the Middle East, and adjusted operating expenses of $19 million–$19.5 million.
The glaucoma model is becoming more utilization-led: higher probe volumes, increased ASPs, reimbursement support from Medicare LCDs, and targeted selling through MedScout could create better recurring revenue than relying on console placements.
The planned headquarters relocation and multiyear migration to lower-cost contract manufacturers could improve gross margins in 2027, but execution depends on maintaining regulatory registrations and supply continuity during the transition.
The retina recovery depends materially on international regulatory and reimbursement progress, including expected European MDR approval for PASCAL in the first half of 2027 and the UK MicroPulse registry.
Competitive pressure remains a structural concern in new Cyclo G6 console placements, particularly in Germany and Austria. The call provided evidence of probe adoption but not proof that the company is winning enough new platforms to expand the installed base at an attractive rate.
The quarter's revenue decline was concentrated in retina, yet management characterized the issues as temporary without quantifying how much of the $1.5 million year-over-year retina shortfall will recover or when China and other regulatory constraints will normalize.
Meeting the unchanged $51 million–$53 million outlook requires a substantial second-half acceleration after first-half weakness, while management simultaneously warned of continued regional ordering choppiness and a likely next-quarter decline in markets that stocked up.
The cash position remains thin at $4.7 million. Safety-stock investment, relocation costs, regulatory blackout periods, and inventory normalization could leave limited room for further execution setbacks.
System sales declined nearly by half year over year, and management attributed the weakness partly to persistent competition rather than only timing. The Q&A did not establish that the competitive issue is temporary.
Adjusted EBITDA deteriorated from positive $21,000 to a loss of $400,000 despite lower operating expenses, so the claimed financial inflection has not yet translated into sustainable earnings improvement.
