Strong execution, but FY27 earnings visibility is insufficient for a clean buy.

Latest call · FY 2026

Hold: FY26 revenue rose 4.2% to $18.6 billion, underlying EBIT grew 11.8% and underlying NPAT 22.9%, while free cash flow was broadly flat at $697 million.

The bull case is Australia-led margin expansion, 1.83x funding-group leverage and a 13.8% dividend increase; the tension is that FY27 EBIT growth is only described as incremental while $10 million-$15 million of additional IT and transformation opex, wage pressure and NHS funding risk remain.

Near term

Australia enters FY27 with 11 newly planned theatres and cath labs, but management must fill recently added capacity while absorbing $10 million-$15 million of additional IT and transformation opex.

The National Capital acquisition is expected to be EPS accretive in its first 12 months, but it will consume $251 million and add to interest expense.

UK hospital performance depends on NHS activity plans, the 1.24% tariff guidance and continued private-pay growth; Elysium remains a turnaround rather than a proven growth asset.

Longer term

Australia's structural upside is higher-acuity mix, theatre utilisation, procurement and insurer contracts with indexation linked to sector metrics; only 48% of private-health-insurance revenue is currently covered by those arrangements.

The proposed Ramsay Santé separation could simplify the group and improve capital focus, but requires the 24 November 2026 shareholder vote and leaves the European business exposed to French funding pressure before separation.

Margin durability depends on offsetting wage inflation through pricing, procurement and agency reductions; revenue-cycle management benefits are not expected to materially reach the P&L until FY28 and beyond.

Red flags

Management declined to quantify FY27 Australian EBIT growth or provide a specific margin target, despite analysts repeatedly testing whether FY26 momentum can be repeated.

Free cash flow was flat year on year despite a 46% improvement in funding-group operating cash flow, with working-capital volatility in Ramsay Santé and higher financing costs still relevant.

The group revised its net-zero Scope 1 and 2 target from 2040 to 2050, citing supplier and commercial constraints.
