Latest call · Q4 2026

Hold rather than buy aggressively:

Q4 was strong, with revenue up 9% to roughly $1.46 billion, non-GAAP EPS up 16% to $2.95, and gross margin up 90 bps to 62.3%. However, FY27 reported guidance of $5.75-$5.85 billion revenue and $12.00-$12.25 EPS implies only 5%-7% revenue and 7%-10% EPS growth, while Astral removes $75 million of revenue and inflation is beginning to outrun productivity. The core business remains attractive, but the call did not fully resolve price-versus-volume, competitive durability, or the timing of next-generation devices.

Near term

Astral sales will be suspended throughout FY27, creating a disclosed $75 million revenue and approximately $0.15 EPS headwind; management says the $42 million field-action provision is the total estimated cost, but execution and patient-safety risks remain.

Gross margin is expected to contract slightly year over year in Q1 before improving later in FY27 as modest price increases and distribution-network productivity ramp; Q1 therefore offers limited near-term margin support.

The MatrixCare divestiture is expected around September 1, 2026. The planned $450 million accelerated repurchase depends on closing, while transition-service agreements are expected to offset stranded costs initially.

Core sleep devices grew 8% constant currency in the Americas and globally, but management declined to quantify the contribution from price versus volume despite direct analyst questioning.

Longer term

ResMed has a credible underpenetration and adherence thesis: AirTouch N30i showed 6% higher 90-day compliance than its silicone equivalent, and GLP-1/PAP users showed higher starts and resupply rates. These are promising indicators, but management has not translated them into a quantified incremental market opportunity.

The company is concentrating on sleep, breathing, and connected home healthcare after selling MatrixCare, with R&D up 22% and continued investment in next-generation devices, masks, and AI workflow tools. Execution quality and returns on these investments will matter more than the broad digital-health narrative.

The AirSense 11 rollout into China and additional AirCurve launches provide runway, but management refused to discuss the timing or capabilities of a successor platform despite an analyst explicitly raising the five-year age of AirSense 11.

Management claims a leading competitive position and says fabric masks are changing the basis of competition, but the Q&A did not test share trends, rival product response, or whether the 6% adherence advantage persists at scale.

FY27 capital returns are targeted above $1.85 billion, including $1.5 billion of repurchases and a 10% dividend increase. This improves shareholder support, but buybacks should not obscure the relatively moderate underlying growth outlook.

Red flags

Management repeatedly emphasized 'meet and beat' ambitions but provided little quantification of the underlying drivers of the 5%-7% growth guide, particularly volume versus price and device versus mask growth.

Inflation in electronic components and freight is now outpacing visible productivity in Q1. The full-year margin improvement depends on benefits that build in Q2-Q4 and on modest price increases that management would not quantify.

The Astral provision is described as all-in and conservative, yet management also called the recall complex and noted it affects a higher device population than prior accruals. The adequacy of the $42 million estimate remains an execution risk.

MatrixCare and Noctrix create reported EPS dilution of approximately $0.30 and $0.20, respectively, while Noctrix integration economics were not quantified. Management's 12%-14% core EPS growth framing excludes these effects and is less directly comparable to reported shareholder returns.

Analyst pushback on AirSense lifecycle, geographic device growth, pricing, and OpEx produced limited disclosure; management would not provide a next-generation launch timetable or detailed spending framework.

The call cited wearables, GLP-1s, and generative AI as demand catalysts, but evidence remains early: the Oura funnel involved approximately 13,000 referred users, and MyAir's 1.5 million inquiries were not tied to revenue or retention outcomes.
